Why titration exists

The dose-limiting effects of GLP-1 agonists — nausea, vomiting, constipation, early satiety — are largely a function of the rate of change rather than the absolute dose. Tolerance to the gastrointestinal effects develops over weeks at a given dose, which is why the labelled schedules hold each step for a fixed period before increasing.

Escalating faster than the label is the most common self-directed modification, and it is also the most common reason people abandon treatment entirely. The schedule is not conservative padding; it is the reason the trials achieved the adherence they did.

How the timeline works

Each product has its own step sequence — a starting dose, a series of increases at fixed intervals, and a maintenance dose that continues open-ended. The chart renders these as a step function, so a dose change appears as a vertical jump rather than a slope, which is what actually happens.

Oral GLP-1s titrate differently

Orforglipron (Foundayo), approved 1 April 2026, is the first oral GLP-1 for weight management without food or water restrictions — a practical difference from oral semaglutide, which must be taken fasted with a small sip of water and a 30-minute wait.

Its ladder runs 0.8 mg → 2.5 mg → 5.5 mg → 9 mg → 14.5 mg → 17.2 mg, with each step held for at least 30 days. That is a longer hold than the four-week steps used by the weekly injectables, and the full titration to maximum dose therefore takes around five months.

Because the label specifies days rather than weeks, the timeline here renders each step as four weeks — very slightly shorter than 30 days. The chart therefore runs marginally ahead of the label rather than behind it.

When the schedule should slow down

Labelled schedules are a default, not a mandate. If side effects at a given step have not settled by the time the next increase is due, holding at the current dose for an additional interval is a recognised and common clinical adjustment. Some people never reach the top maintenance dose and do perfectly well below it.
