The short answer

Evidence tier: 1 — regulator labels and large fracture RCTs for PTH analogues. Tier 2–4 for everything else.

Yes, but the peptides with real fracture data are prescription drugs. Teriparatide (Forteo) and abaloparatide (Tymlos in the US, Eladynos in the EU and UK) are injectable peptides that copy parathyroid hormone. They build new bone. In large randomized trials they cut new spine fractures by roughly two-thirds or more in postmenopausal women with osteoporosis. Calcitonin is a third peptide. It is still approved in the US, but it is now a last resort.

The "research peptides" marketed for bone are a different story. MOTS-c and BPC-157 have animal data only. In July 2026 an FDA advisory committee narrowly voted to support compounding MOTS-c, with obesity and osteoporosis among the uses discussed. That vote doesn't make it an approved drug. Collagen peptides taken by mouth have one well-run randomized trial showing small bone density gains.

GLP-1 weight-loss drugs are peptides too, and they may lower bone density as weight comes off.

The key points:

  • Teriparatide: 65% fewer new vertebral fractures than placebo at the approved 20 µg dose (Neer et al. 2001).
  • Abaloparatide: fewer vertebral and non-vertebral fractures over 18 months, and less high blood calcium than teriparatide (Miller et al. 2016).
  • MOTS-c: prevented bone loss in mice whose ovaries were removed. No published human bone studies.
  • BPC-157: helped bone defects heal in rabbits. No human bone or osteoporosis trials.
  • Collagen peptides (5 g/day): small gains in spine and hip bone density over 12 months in one RCT of 131 women (König et al. 2018).
  • GLP-1s: randomized trials show small spine bone density losses with semaglutide and liraglutide 3.0 mg. In one trial, adding exercise to liraglutide prevented this.

How do teriparatide and abaloparatide build bone?

Evidence tier: 1 — phase 3 fracture RCTs and FDA/EMA labels.

Most osteoporosis drugs, such as bisphosphonates and denosumab, slow bone breakdown. Parathyroid hormone works differently when given in short daily pulses. It switches on bone-building cells. Both drugs are short peptides injected under the skin once a day.

Teriparatide is the first 34 amino acids of human PTH. In the pivotal trial, 1,637 postmenopausal women with earlier spine fractures were followed for a median of 21 months (Neer et al. 2001):

  • New vertebral fractures: 14% on placebo vs 5% on 20 µg (relative risk 0.35)
  • New non-vertebral fragility fractures: 6% vs 3%
  • Spine bone density: 9 percentage points higher than placebo

Abaloparatide is a modified version of PTH-related protein. The ACTIVE trial enrolled 2,463 women (average age 69) for 18 months (Miller et al. 2016). It reduced vertebral and non-vertebral fractures against placebo. High blood calcium was less common than with open-label teriparatide (3.4% vs 6.4%).

What do the labels say about limits and safety?

  • Teriparatide (US): approved for postmenopausal women with osteoporosis at high fracture risk. The osteosarcoma boxed warning was removed in 2020 after large registry studies found no excess bone cancer (Krege et al. 2022). Use beyond 2 years in a lifetime should be considered only if fracture risk stays high (FDA label).
  • Abaloparatide (US): use for more than 2 years in a lifetime is not recommended (FDA label).
  • EU/UK: teriparatide (Forsteo and biosimilars) is authorised. Abaloparatide (Eladynos) received EU authorisation in December 2022 (EMA). NICE recommended it in England in August 2024 under TA991 (Pharmaceutical Journal).

Order matters. The Menopause Society's osteoporosis statement lists PTH-receptor agonists among approved options. It recommends bone-building drugs as first treatment for women at very high fracture risk (NAMS 2021). After a course ends, specialists usually follow with a drug that slows bone breakdown, so the gains aren't lost.

With HRT: two small randomized trials added PTH(1-34) to ongoing HRT. Spine bone density rose about 13%, and vertebral fractures fell compared with HRT alone (Lindsay et al. 1997; Cosman et al. 2001). Our HRT and peptides article covers combinations more broadly.

Is calcitonin still used for osteoporosis?

Evidence tier: 1 — regulator decisions and labels.

Calcitonin (from salmon) is a 32-amino-acid peptide that slows bone breakdown. Its role has shrunk:

  • EU (2012): the EMA said calcitonin nasal spray should no longer be prescribed for osteoporosis. Long-term trials showed a 0.7–2.4% higher cancer rate than placebo, and the drug did little to prevent fractures (EMA).
  • US: the Miacalcin injection label keeps an indication for women more than 5 years past menopause. It notes "fracture reduction efficacy has not been demonstrated" and reserves the drug for patients who can't use alternatives (label).

What is romosozumab, and is it a peptide?

Evidence tier: 1 — FDA label.

No. Romosozumab (Evenity) is a monoclonal antibody, a large protein that blocks sclerostin. It's worth knowing about because, like the PTH analogues, it builds bone. It is given monthly for a maximum of 12 doses. Its FDA label carries a boxed warning about heart attack, stroke and cardiovascular death (Evenity label). The signal came from the ARCH trial against alendronate (Saag et al. 2017).

What does the evidence show for MOTS-c and bone?

Evidence tier: 4 — mouse and cell studies. No published human bone data.

MOTS-c is a 16-amino-acid peptide encoded in mitochondrial DNA. In mice with their ovaries removed, a standard model of postmenopausal bone loss, daily MOTS-c injections for 12 weeks reduced bone loss on micro-CT scans. It also blocked the formation of bone-resorbing cells through AMPK (Ming et al. 2016).

What happened at the FDA in 2026:

  • In April 2026, the FDA removed MOTS-c and 11 other peptides from its Category 2 list of compounding substances with safety concerns. The removal followed withdrawn nominations and did not make them compoundable (Goodwin).
  • On 23 July 2026, the Pharmacy Compounding Advisory Committee reviewed MOTS-c for obesity and osteoporosis. It voted 7–5 with 2 abstentions to support adding it to the 503A bulks list (PharmExec).
  • Public Citizen testified that there are no published human studies of MOTS-c. The only human data come from a small trial of an analogue (CB4211) whose development stopped (Public Citizen).
  • The vote is advisory. Any change needs FDA rulemaking, and the bulks list had not been amended at the time of writing (Buchanan Ingersoll & Rooney).

Our MOTS-c evidence guide covers the metabolic research.

Does BPC-157 help bone?

Evidence tier: 4 — animal fracture and bone-defect studies. No human bone trials.

In rabbits with a surgically created gap in the bone, BPC-157 improved healing. The effect was comparable to bone marrow or bone graft in some measures (Sebečić et al. 1999). A 2025 systematic review found 35 preclinical studies and 1 small human study (knee pain, not bone). It reported no clinical safety data (Vasireddi et al.).

Healing a fracture or a surgical defect is not the same as treating osteoporosis, which is bone that has become thin and fragile everywhere. No study has tested BPC-157 in osteoporosis, in animals or humans. In July 2026 the advisory committee voted 8–6 to support compounding BPC-157, for ulcerative colitis, not bone. The same caveats apply: the vote is advisory and bulks list status hasn't changed. Our BPC-157 evidence review has more detail.

Do GH-releasing peptides build bone?

Evidence tier: 2 — one 18-month RCT of MK-677 in postmenopausal women with low bone density.

Growth hormone speeds up bone turnover. Merck tested MK-677 (ibutamoren) with and without alendronate in 292 women aged 64–85 with low hip bone density (Murphy et al. 2001):

  • IGF-1 rose by about 40%
  • MK-677 plus alendronate raised femoral neck bone density more than alendronate alone (4.2% vs 2.5%)
  • There was no extra gain at the spine, total hip or whole body
  • GH-related side effects appeared. The authors called the limited benefit "a concern" when weighed against those side effects

A small trial of a GHRH analogue in adults aged 55–71 found no change in bone density after 16 weeks (Khorram et al. 1997). We found no bone data in postmenopausal women for tesamorelin, sermorelin, ipamorelin or CJC-1295.

Do oral collagen peptides improve bone density?

Evidence tier: 2 — one double-blind RCT plus an open-label extension, from a single research group.

These are digested food proteins, not injectable signalling peptides. In a 12-month double-blind trial, 131 postmenopausal women with reduced bone density took 5 g of specific collagen peptides or placebo every day (König et al. 2018):

  • Spine T-score: +0.10 vs −0.03 (p = 0.03)
  • Femoral neck T-score: +0.09 vs −0.01 (p = 0.003)
  • A bone-formation marker (P1NP) rose. A breakdown marker (CTX) rose in the placebo group

An open-label follow-up of 31 women reported continued gains over about 4 years (Zdzieblik et al. 2021). One co-author co-invented collagen-peptide patents. These changes are much smaller than with prescription drugs, and fracture outcomes were not tested.

Can GLP-1 drugs lower bone density?

Evidence tier: 2 — small RCTs and meta-analyses. No fracture trials of semaglutide 2.4 mg or tirzepatide.

Fast weight loss from any cause tends to reduce bone density. For GLP-1s:

  • Liraglutide 3.0 mg vs exercise: after diet-induced weight loss, liraglutide alone lowered hip and spine bone density more than exercise alone, despite similar weight loss. Liraglutide plus exercise kept bone density at placebo levels (Jensen et al. 2024).
  • Semaglutide: one low-risk-of-bias RCT (n = 64) found lower spine and whole-body bone density and higher bone breakdown markers. A 2026 systematic review rated the overall evidence certainty as very low (Lai et al. 2026).
  • Meta-analysis, mostly older GLP-1s: a small spine bone density drop (−0.02 g/cm²), no hip change. No eligible placebo-controlled trials of semaglutide 2.4 mg or tirzepatide (Safargar et al. 2026).
  • Broader meta-analysis (60 studies): no significant effect on bone density or fractures after adjusting for confounders, but consistent lean mass loss (Beaudart et al. 2026).

Mayo Clinic researchers point out that postmenopausal women already lose bone because of low estrogen. Fracture data in this group remain inconsistent (Castaneda et al. 2026). A bone density scan before or during major weight loss is a question to raise with your doctor.

Limitations of the evidence

Evidence tier: — summary of the evidence gaps above.
  • MOTS-c and BPC-157 have animal and cell data only, with no human bone studies. BPC-157 has never been tested in osteoporosis.
  • The July 2026 committee votes were advisory, not drug approvals.
  • The collagen data come from one trial by a single research group, and fractures weren't measured.
  • The MK-677 benefit was limited to the femoral neck in one trial.
  • There are no fracture trials for semaglutide 2.4 mg or tirzepatide, meta-analyses disagree, and one 2026 review rated evidence certainty very low.

The bottom line

The peptides proven to cut fractures after menopause, teriparatide and abaloparatide, are prescription drugs. A specialist chooses them based on your fracture risk. MOTS-c and BPC-157 have animal data only, and BPC-157 has none in osteoporosis. Neither has human bone trials, and an advisory vote is not an approval. Collagen peptides show small bone density gains in one trial. If you're on a GLP-1, protecting bone through exercise and monitoring is a discussion to have with your physician.

Frequently asked questions

What is the strongest peptide treatment for osteoporosis?
Teriparatide and abaloparatide, both daily injections that act on the PTH receptor to stimulate bone formation. In phase 3 trials they cut new spine fractures substantially compared with placebo. They're prescription drugs for women at high fracture risk. Abaloparatide is not recommended beyond 2 years in a lifetime; teriparatide beyond 2 years is considered only if fracture risk stays high.
Did the FDA approve MOTS-c for osteoporosis?
No. In July 2026 an FDA advisory committee voted 7–5, with 2 abstentions, to support adding MOTS-c to the list of substances pharmacies may compound. Obesity and osteoporosis were the uses reviewed. The vote is non-binding, needs FDA rulemaking to take effect, and isn't a drug approval. No human bone studies of MOTS-c have been published.
Can BPC-157 reverse bone loss?
There's no evidence it does. BPC-157 has improved healing of surgically created bone defects in animal studies, such as rabbits. No study in animals or humans has tested it in osteoporosis, which is generalized bone thinning rather than a single injury.
Do collagen supplements help bone density after menopause?
One 12-month double-blind trial of 131 postmenopausal women found that 5 g/day of specific collagen peptides gave small increases in spine and hip T-scores compared with placebo. The effect is much smaller than with prescription drugs, came from a single research group, and fractures weren't measured.
Does Ozempic or Mounjaro cause bone loss?
Some randomized trials show small decreases in spine bone density with semaglutide and liraglutide 3.0 mg as weight falls. Meta-analyses differ, and there are no fracture trials for semaglutide 2.4 mg or tirzepatide. In one trial, adding structured exercise to liraglutide kept bone density at placebo levels.
Is romosozumab (Evenity) a peptide?
No. Romosozumab is a monoclonal antibody that blocks sclerostin. Like the PTH analogues, it builds bone. It's limited to 12 monthly doses and carries a boxed warning for heart attack, stroke and cardiovascular death.
