How do TRT and peptides work together, and what are the interactions?

TRT and peptides do different jobs. Testosterone restores androgen levels; peptides cover what it doesn't — fertility and testicular function (hCG, kisspeptin), body composition (GH secretagogues), and libido when TRT isn't enough (PT-141). The central interaction: TRT shuts down your own production, which is exactly where the fertility peptides come in.

Which peptides actually help with sexual function and libido — and how do they differ from Viagra-class drugs?

PT-141 (bremelanotide) is the only FDA-approved sexual-health peptide, acting on brain melanocortin pathways for arousal — a different mechanism than Viagra-class vascular drugs. Kisspeptin and oxytocin have early human evidence for libido and bonding. Most other sexual-health peptide claims are mechanism plus marketing.

How do you preserve fertility and testicular function while on TRT?

Taking testosterone shuts down your own production: the brain stops signaling the testes, which shrinks them and impairs sperm production — a leading, often-preventable cause of male infertility. The fix is to keep the testes working with hCG (which mimics LH), planned from the start, with kisspeptin as an emerging upstream option.

Which peptides actually help women with low libido or sexual dysfunction, and what's the evidence?

PT-141 (Vyleesi) is FDA-approved for premenopausal HSDD — the only sexual-health peptide with female-specific approval. Kisspeptin shows promising early human evidence for female desire. Oxytocin has mixed data. The female sexual-health peptide space is actually better-evidenced than the male space because PT-141's approval was for women.

Will kisspeptin and other emerging peptides actually help female sexual health, and when?

Yes — eventually. Kisspeptin targets the upstream hypothalamic GnRH/sexual-arousal circuit, distinct from PT-141 (dopaminergic) and flibanserin (serotonergic). Phase 2 data shows brain-activation and subjective response improvements, including in postmenopausal women. Not currently a clinical option. Realistic availability 2027-2030. Today's options remain PT-141, flibanserin, off-label testosterone, oxytocin.
