The short answer
Evidence tier: 1 — reflux, dyspepsia and burping rates come from the placebo-controlled adverse-reaction tables in the current FDA labels for Zepbound, Mounjaro, Wegovy and Ozempic.
Heartburn and acid reflux are recognised side effects of tirzepatide (Mounjaro, Zepbound) and semaglutide (Ozempic, Wegovy). In the weight-loss trials behind the labels, gastroesophageal reflux disease (GERD) was reported in 4–5% of people on Zepbound versus 2% on placebo, and in 5% on Wegovy versus 3% on placebo. Indigestion (dyspepsia) was roughly twice as common again.
The main points:
- The likely driver is slower stomach emptying, strongest after the first dose and after dose increases.
- It is mostly a dose-escalation problem that fades over time, according to the labels.
- Acid-reducing medicines are a prescriber conversation, especially if symptoms are new, severe or persistent.
- Some symptoms are not heartburn. Chest pain, trouble swallowing, repeated vomiting or severe upper-abdominal pain need medical assessment.
For the wider picture, see GLP-1 side effects and how to manage them.
Why does Mounjaro cause heartburn?
Evidence tier: 2 — the gastric-emptying effect is stated in the prescribing information and measured in pharmacology studies; its direct link to reflux symptoms is inferred rather than tested.
GLP-1 receptor agonists slow the rate at which food leaves the stomach. The Zepbound label states that tirzepatide delays gastric emptying and that "the delay is largest after the first dose and this effect diminishes over time." The Wegovy label states that semaglutide also delays gastric emptying. A Lilly study found a single tirzepatide dose delayed emptying about as much as selective GLP-1 drugs, with the effect waning on repeated dosing in healthy volunteers (Urva et al., 2020). A 20-week semaglutide 2.4 mg study found no measurable delay by week 20 (Friedrichsen et al., 2021).
A stomach that stays full for longer puts more volume and pressure below the valve between stomach and oesophagus, giving acid more chance to move upward. That is felt as burning behind the breastbone, a sour taste or food "sitting high". A portion that was normal before treatment can now overfill a slow-emptying stomach, and fatty food empties slowly even without a GLP-1. Nausea and burping often travel with reflux; sulfur-smelling burps have their own guide: Ozempic sulfur burps.
Because the emptying effect fades over weeks, the commonly described pattern (worse after starting or after each step up, then easing) fits the mechanism. No trial has tracked heartburn against measured gastric emptying, so this remains an inference.
How common is acid reflux on tirzepatide and semaglutide?
Evidence tier: 1 — figures copied from the adverse-reaction sections of FDA labels revised in 2026 (Zepbound 08/2026, Mounjaro 08/2026, Wegovy 02/2026, Ozempic 05/2026).
| Product (trial population) | Placebo | Lower dose | Middle dose | Top dose |
|---|---|---|---|---|
| Zepbound, weight trials: GERD | 2% | 4% (5 mg) | 4% (10 mg) | 5% (15 mg) |
| Zepbound: dyspepsia | 4% | 9% | 9% | 10% |
| Mounjaro, type 2 diabetes: GERD | 0.4% | 1.7% (5 mg) | 2.5% (10 mg) | 1.7% (15 mg) |
| Mounjaro: dyspepsia | 3% | 8% | 8% | 5% |
| Wegovy 2.4 mg injection, weight trials: GERD | 3% | — | — | 5% |
| Wegovy: dyspepsia | 3% | — | — | 9% |
| Ozempic, type 2 diabetes: GERD | 0% | 1.9% (0.5 mg) | 1.5% (1 mg) | — |
Rates are higher in the weight-management trials than in the diabetes trials, partly because doses and populations differ. The excess over placebo is a few percentage points, so most people on these drugs do not report reflux. "GERD" in a trial table is an adverse-event term, not a diagnosis confirmed by testing.
Burping (eructation) ran at 4–5% on Zepbound versus 1% on placebo, and 7% on Wegovy versus under 1%. For retatrutide, which is not approved, the phase 2 trial reported gastrointestinal events as the most common adverse events, dose-related and mostly mild to moderate (Jastreboff et al., 2023). We did not find a verified reflux-specific rate to quote.
When does GLP-1 heartburn start and how long does it last?
Evidence tier: 2 — timing comes from label statements and a pooled trial analysis of all gastrointestinal events; no source we verified reports the duration of reflux on its own.
For Zepbound and Mounjaro, "the majority of nausea, vomiting, and/or diarrhea events occurred during dose escalation and decreased over time." For Wegovy, gastrointestinal reactions "were most frequently reported during dosage escalation." A pooled analysis of the STEP 1–3 semaglutide trials found that 98.1% of gastrointestinal events were mild to moderate and that they were typically transient and clustered during or shortly after dose escalation (Wharton et al., 2022).
So the first weeks of a new dose are the likeliest time for trouble, one reason titration steps last at least four weeks (see the titration schedule guide). Many people say the first two or three days after a shot are worst, though that is user-reported rather than trial-measured. Reflux that persists for weeks at a stable dose is different from a short flare after a step up and is worth raising with the prescriber.
What helps with heartburn on a GLP-1?
Evidence tier: 3 — standard reflux self-care measures used in general practice, applied to GLP-1 physiology; none has been tested in a GLP-1-specific trial.
General measures, not a treatment plan:
- Eat smaller meals, slowly, and stop at the first sign of fullness. Fullness arrives earlier on a GLP-1, and overshooting it is a common trigger.
- Go easier on fatty, fried and very rich food, especially in the days right after an injection.
- Leave two to three hours between the last meal and lying down, and consider raising the head of the bed if symptoms are worse at night.
- Notice personal triggers such as alcohol, fizzy drinks, coffee, chocolate, mint or spicy food. See alcohol and GLP-1s.
- Sip fluids between meals if large drinks with food make you overfull, while keeping total intake up.
- Keep a short log of dose, injection day, meals and symptoms to bring to the prescriber.
Food choices are covered in what to eat on a GLP-1; if nausea or slow bowels dominate, see GLP-1 nausea and GLP-1 constipation.
Do not skip, split or reduce doses on your own to control reflux. Staying on a lower step for longer, or stepping back down, is a reasonable thing to ask about, and the decision belongs with the prescriber.
Can I take a PPI or antacids on Mounjaro?
Evidence tier: 3 — label drug-interaction sections and one pharmacokinetic finding; there are no trials testing acid-reducing medicines for GLP-1-related reflux.
The labels we read do not list proton pump inhibitors (PPIs), H2 blockers or antacids as contraindicated with tirzepatide or semaglutide. The Wegovy label notes that semaglutide levels were not meaningfully changed by omeprazole with the oral tablet, a pharmacokinetic finding rather than evidence that omeprazole treats GLP-1 reflux.
The labels do warn that delayed gastric emptying "may impact absorption of concomitantly administered oral medications," with monitoring advised for narrow-range drugs such as warfarin. Antacids can also alter absorption of some medicines. So adding an acid-reducing product is a medicines review, not just a symptom fix.
Questions worth taking to a prescriber or pharmacist:
- Do my symptoms need assessment first, given how long or severe they are?
- Which fits: an occasional antacid or alginate, an H2 blocker, or a short PPI course, and for how long?
- Does it need spacing from levothyroxine, iron or an oral contraceptive? Tirzepatide carries a specific oral-contraceptive warning; see GLP-1s and contraception.
- On oral semaglutide, how does it fit the label's routine of no more than 4 ounces of water and a 30-minute wait before food, drinks or other medicines? See oral GLP-1s.
When is heartburn on a GLP-1 a red flag?
Evidence tier: 1 — warning signs drawn from the Warnings and Precautions sections and Medication Guides of the Zepbound, Ozempic and Wegovy labels, plus standard emergency criteria for chest pain.
Get urgent care or contact your prescriber promptly for:
- Chest pain, pressure or tightness, especially with breathlessness, sweating, or pain spreading to the arm, jaw or back.
- Trouble swallowing, pain on swallowing, or food sticking.
- Vomiting that will not stop, or being unable to keep fluids down. The labels warn that fluid loss from vomiting or diarrhoea can cause acute kidney injury.
- Vomiting blood or material like coffee grounds, or black, tarry stools.
- Severe abdominal pain that will not go away, sometimes spreading to the back, with or without vomiting. The labels describe this as a possible sign of pancreatitis.
- Upper-right abdominal pain, fever, yellowing of the skin or eyes, or clay-coloured stools, which the labels list as possible gallbladder symptoms. See GLP-1s, gallbladder and pancreatitis.
A database study of weight-loss GLP-1 users reported higher risks of pancreatitis, bowel obstruction and gastroparesis than with bupropion-naltrexone (Sodhi et al., 2023). The events are uncommon, but severe or persistent symptoms should not be written off as reflux; the labels also say these drugs are not recommended in severe gastroparesis.
If you are booked for surgery, endoscopy or sedation, tell the team you take a GLP-1. The labels warn of rare pulmonary aspiration under anaesthesia because food can remain in the stomach despite fasting.
What do users report?
Evidence tier: 4 — keyword counts from an anonymised Reddit dataset; self-selected posts, no verification of drug, dose or diagnosis.
The site's anonymised Reddit dataset holds 3,710 posts from 7 June to 5 November 2026. Keeping posts tagged tirzepatide, semaglutide or retatrutide, or posted in r/Mounjaro, r/Semaglutide or r/Retatrutide, and removing duplicates left 2,642 posts (8 June to 5 November). A case-insensitive keyword search for heartburn, reflux, GERD, LPR, indigestion, acidic and common acid-medicine names matched 62 posts (2.3%). For comparison, "nausea" matched 236.
Of the 62, 50 were tagged tirzepatide, 15 semaglutide and 3 retatrutide (some carried more than one tag). 28 also mentioned nausea or vomiting, 17 named an antacid, alginate, H2 blocker or PPI, and 8 mentioned sulfur burps. Recurring themes, paraphrased: reflux starting in the first weeks or after a dose increase; fatty or normal-sized meals as triggers; night-time symptoms disturbing sleep; flares in people with long-standing reflux; questions about daily PPIs versus occasional antacids; and a few people reporting less reflux after stepping down a dose with their prescriber.
These counts show what people talk about, not how often reflux happens.
Limitations
This is an educational guide, not medical advice.
- Label rates come from trial populations that may differ from you in dose, diabetes status and other medicines.
- No verified source reports how long reflux lasts separately from other gastrointestinal effects.
- The mechanism is inferred: delayed emptying is measured, its role in reflux symptoms is not.
- Keyword matching misses some Reddit posts and includes some where reflux is incidental.
The bottom line
Heartburn on Mounjaro, Zepbound, Ozempic or Wegovy is real but affects a minority: label reflux rates sit a few points above placebo, with indigestion a little more common. Slower stomach emptying, strongest early and after each dose increase, is the likely reason, so the problem is often concentrated in the escalation phase.
Smaller, slower, lower-fat meals and an earlier last meal are the first things to try. Whether to add an antacid, H2 blocker or PPI, and how it fits around your other medicines, is a question for the prescriber or pharmacist. Chest pain, trouble swallowing, persistent vomiting or severe abdominal pain are not things to treat at home.
